The problem
When healthcare records sit in separate systems, care teams may need to log into outside portals, request documents and manually gather information. A recent medication change or discharge plan can be difficult to find when it matters. This demonstration explores how integrated access could reduce record chasing and make the next step clearer, while clinicians remain responsible for confirming medication use, clarifying decisions and coordinating follow-up.
Two journeys, one patient
- From primary care and cardiology to the emergency department. Follow fictional Avery Morgan as an ED clinician opens available outside notes, notices a medication-dose discrepancy, checks the history with Avery and speaks directly with cardiology before documenting the ED plan.
- From the emergency department back to primary care and home. Separate the discharge alert from later clinical records. Watch the PCP review the plan and an RN confirm instructions with Avery. The next step has a named owner; future appointments remain future care.
The return journey includes a CCD (Continuity of Care Document), discharge summary and patient instructions when available in this fictional scenario. Their availability depends on source sharing and authorized access; this is not an automatic document package for every ED visit.
Published foundation · Rejoin possibilities
Viewing a record and acting on it are separate.
Florida HIE's published services describe inContext access to available outside information within an integrated EHR. Its organizational guidance explains participation and access. This demo does not confirm any organization's implementation.
Rejoin's narrative summaries, discrepancy detection, task routing, clinician-reviewed chart updates and custom handoff workflow are envisioned enhancements. The demonstration uses authored records and scripted actions; the clinicians make the fictional care decisions.

Choose Play story for a 78-second scripted tour, or use Next and Previous step at your own pace. Pause, inspect a source note or explore a missing-record or pending-callback scenario. No typing is required. The 78 seconds describe the narration, not the time needed to deliver care.
